Thank you for your interest in receiving your future account statements through C.U. OnLine, PFCU’s Home Banking Service. (To sign up for C.U. OnLine and obtain a PIN, complete the appropriate request form first). We are pleased to make this service available to you at no additional cost. This service will let you obtain your statement(s) faster, and it avoids the risk that your statement(s) will be lost in the mail or stolen from your mailbox. Please complete and sign this form to receive account statement(s) for all of your accounts through C.U. OnLine. For joint accounts, both joint tenants must sign this form. You understand that your statements will include information concerning both your Share/Deposit Accounts and Loan/Credit Card Accounts with the Credit Union, and may include disclosures required under the Truth-In-Lending Act, the Electronic Funds Transfer Act, the Truth-In-Savings Act, and/or similar consumer disclosure laws (“consumer disclosures.”) You agree to the receipt of these consumer disclosures in electronic format as described herein.

Please provide us with any changes in your e-mail address in the future. You can do this through C.U. OnLine or by writing to the Credit Union.

By requesting your statements electronically we will suspend sending you the statements by mail unless you notify us to the contrary. If after using the Home Banking Service to obtain your statements you decide you want to return to receiving your statements by mail, simply contact us and request this change.

You will be notified by email when your statement(s) are available. If the email notice that the statement is available is returned, a reminder will be sent. If the statement is not retrieved, we will take reasonable steps to attempt redelivery. You can gain access to your account statements for 90 days by using any type of computer that is linked to the internet. You need to use as a browser either Microsoft Explorer version 4.04 or higher. You will also need to run an Acrobat Reader version 4.0 or higher to view your statements. The C.U. OnLine site includes links to sites from which you can download all of the required software. You will need a printer if you want to create a paper-copy of your statements. By your request to receive your statements electronically, you are telling the Credit Union that you have printing capability. Periodically, you may receive promotional and/or legislative information from the Credit Union on its products, services or interest rates.

I/We hereby agree to receive my/our account statements, with all required consumer disclosures, through C.U. OnLine. A copy of this Electronic Statement and Disclosure Agreement may be printed out from our website www.parsonsfcu.org.
